These are income based grants awarded to families. The screenshot below shows where you can find the application on your FACTS account dashboard. There is an application fee. All applications need to be completed and submitted by December 17th, 2021. You will need your prior year taxes and W2 forms to complete the application. Go to factsmgt.com and login.
